First Visit

Your initial appointment may consist of a consultation or treatment depending on the type of procedure required and the issues you are experiencing. If patients have provided insurance benefits prior to the appointment, the insurance benefits will be verified to determine what should be covered. Should patients have a complex medical history or be under a specialist care for some other condition, a consultation may be needed prior to scheduling a procedure.  A consultation appointment may also be required when patients or guardians are anxious, have specific questions for the provider, or are unfamiliar with the oral surgery setting. Dr. Forshey will take the time to explain your diagnosis, treatment options, anesthesia options, and the risks of various procedures.

Whether being seen as a consultation or treatment, we will present the patient with a pre-treatment estimate by our treatment plan coordinator based on the insurance benefits provided prior to the appointment. Should you have insurance, she will let you know what benefits are expected to covered and your estimated out of pocket cost prior to your appointment.  All estimated costs (co-pays and/or deductibles) are due at time of service. Additionally, some procedures may require insurance pre-determination to be completed prior to rendering treatment, and the required information will be gathered and submitted to your insurance company after the initial consultation. When pre-determinations are required,  we will be unable to schedule the procedure until we have received the “Estimate of Covered Services” provided by your insurance.


Please assist us by providing the following information at the time of your appointment:

  • Your referral slip or extraction order from the general dentist or other dental specialist and any x-rays, if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary cards. This will save time and allow us to help you process any claims. 
  • Also, remember to bring a valid state or federally issued photo identification, with your current address and name.  For minor patients, the parent or guardian must provide this information and be present.  

ONLINE REGISTRATION:  You can complete our new patient information online through the secure truForm Patient Registration program at this site prior to your visit. This will make your visit quicker and more organized.  

IMPORTANT:   All patients under the age of 18 years of age MUST be accompanied by a parent or guardian.

If you are having IV sedation/general anesthesia, it is a requirement that you have nothing to eat or drink after midnight  prior to your procedure and you have an escort (over age of 18 yo) that will be driving you home after the procedure.  This escort must remain in the office waiting area until you are ready to leave the office.  

Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g. heart medications, aspirin, blood thinners, chemotherapy, etc.).  If you have a medical condition that requires pre-medication, please contact our office and we will assist you with this requirement.
